soci\u00e9t\u00e9 <\/a> IHSS, D\u00e9partement \u00c9tudes psychanalytiques<\/p>\n L\u2019exp\u00e9rience de ces deux derni\u00e8res d\u00e9cennies nous permet d\u2019affirmer que les r\u00e9ponses purement mat\u00e9rielles ne constituent pas une vraie solution \u00e0 la souffrance de la grande pr\u00e9carit\u00e9 et de l\u2019exclusion. En effet, les causes de l\u2019exclusion sociale sont multiples (ruptures familiales et identitaires ; mouvements migratoires forc\u00e9s \u00e0 cause des guerres, catastrophes naturelles, pers\u00e9cution politique ou religieuse) et ses manifestations sont aussi complexes, concernant des aspects sociaux, \u00e9conomiques, anthropologiques et psychologiques. A partir de ces constats de terrain, un collectif de praticien.e.s (psychologues, \u00e9ducateurs et \u00e9ducatrices, travailleurs et travailleuses du social) travaillant dans la rue aupr\u00e8s de personnes sans abri en \u00cele-de-France a commenc\u00e9 en 2016 une r\u00e9flexion sur la particularit\u00e9 de cette d\u00e9marche et sur la complexit\u00e9 sp\u00e9cifique \u00e0 ce type d\u2019accompagnement social et psychoth\u00e9rapeutique, qui \u00e9volue souvent hors-langage et dans les interstices des institutions. Depuis 2020, ce collectif s\u2019est associ\u00e9 \u00e0 un groupe de chercheurs (Universit\u00e9 Paris Cit\u00e9, Universit\u00e9 Sorbonne Paris Nord, Universit\u00e9 Paris 8, Aix Marseille Universit\u00e9) et d\u2019artistes pour d\u00e9velopper des pratiques capables de r\u00e9pondre \u00e0 cette manifestation contemporaine de la souffrance subjective et sociale. Cette initiative a trouv\u00e9 une continuit\u00e9 gr\u00e2ce \u00e0 l\u2019inscription de ces rencontres et d\u00e9bats \u00e0 la Maison des Sciences de l\u2019Homme (MSH Paris Nord) depuis 2022. Conform\u00e9ment \u00e0 ce qui \u00e9tait attendu, une \u00e9tude corr\u00e9lationnelle (N = 535) et une \u00e9tude exp\u00e9rimentale (N = 370) men\u00e9es aupr\u00e8s de participants fran\u00e7ais ont montr\u00e9 qu’\u00eatre nostalgique du pass\u00e9 de la France favorise les ressentis d’angoisse concernant l’avenir du pays, \u00e0 travers une perception accrue d’anomie soci\u00e9tale dans la soci\u00e9t\u00e9 actuelle. Ces r\u00e9sultats sugg\u00e8rent que : (1) la fa\u00e7on dont nous nous repr\u00e9sentons le pass\u00e9 national fa\u00e7onne nos perceptions de la soci\u00e9t\u00e9 pr\u00e9sente et future ; (2) la nostalgie nationale, en plus d’\u00eatre une strat\u00e9gie de coping face \u00e0 des menaces existentielles comme sugg\u00e9r\u00e9 dans des travaux ant\u00e9rieurs, pourrait \u00e9galement Mme Anne-Claire Dorsemans, psychologue clinicienne et chercheuse, attach\u00e9e \u00e0 la consultation de G\u00e9n\u00e9tique du Groupe Hospitalier Piti\u00e9-Salp\u00e9tri\u00e8re, Institut du Cerveau et de la Moelle (ICM)<\/p>\n
M\u00e9thodes : L\u2019\u00e9tude DIRAGENE, une \u00e9tude prospective observationnelle utilisant des m\u00e9thodes quantitatives et qualitatives avec une approche psychanalytique, a \u00e9t\u00e9 men\u00e9e entre octobre 2020 et mars 2022 en partenariat entre le Laboratoire PCPP et le Centre de R\u00e9f\u00e9rence des Maladies Neurog\u00e9n\u00e9tiques au sein du D\u00e9partement de G\u00e9n\u00e9tique clinique du Groupe hospitalier Piti\u00e9-Salp\u00eatri\u00e8re (Institut du Cerveau et de la Moelle, ICM). Elle a explor\u00e9 les effet de l\u2019\u00e9vocation des DAs et de leurs repr\u00e9sentations associ\u00e9es aupr\u00e8s de patients et de leurs proches concern\u00e9s par des maladies neurod\u00e9g\u00e9n\u00e9ratives graves et rares telles que la Maladie de Huntington, les Ataxies C\u00e9r\u00e9belleuses Autosomiques Dominantes par expansion de polyglutamine et l\u2019Ataxie de Friedreich. Ces pathologies sont d\u2019\u00e9tiologie g\u00e9n\u00e9tique, engendrent une d\u00e9pendance consid\u00e9rable et ne b\u00e9n\u00e9ficient pas de traitement curatif \u00e0 ce jour. Les patients sont donc affect\u00e9s de fa\u00e7on irr\u00e9versible par le d\u00e9clin progressif de leurs capacit\u00e9s cognitives et\/ou motrices au cours de l\u2019\u00e9volution de la maladie, rendant la communication difficile, voire impossible. Les DAs sont alors dans ce contexte une ressource d\u00e9cisionnelle pertinente et importante lors de situations de fin de vie aux tensions \u00e9thiques complexes pour les praticiens, comme pour les familles.<\/em>
Population : Au total, 124 personnes ont particip\u00e9 \u00e0 cette \u00e9tude, 81 patients (ce qui repr\u00e9sente un taux de participation de 34 % pour les patients par rapport au nombre de patients sollicit\u00e9s) et 43 aidants.<\/em>
R\u00e9sultats pr\u00e9liminaires :<\/em>
– Ce dispositif l\u00e9gal est peu utilis\u00e9, peu connu, souvent ignor\u00e9 et\/ou occult\u00e9.<\/em>
– En effet, les entraves pour aborder le sujet de la fin de vie sont nombreuses comprenant \u00e0 la fois le rapport complexe que nous entretenons avec l\u2019id\u00e9e de notre propre mort, la mort de l\u2019autre et nos efforts pour la maintenir \u00e0 distance, \u00e0 l\u2019\u00e9cart de la conscience.<\/em>
– Notons \u00e9galement le tabou soci\u00e9tal autour de la mort, les difficult\u00e9s de communication, les histoires familiales et l\u2019identification aux personnes disparues, la peur de nuire \u00e0 autrui, la difficult\u00e9 \u00e0 choisir le moment opportun pour en parler, le sentiment de ne pas se sentir capable de soutenir cette discussion ou encore de ne pas savoir qui devrait l\u2019aborder ni comment.<\/em>
Conclusion : L\u2019\u00e9tude DIRAGENE renseigne sur les enjeux que posent les DAs pour la personne concern\u00e9e, son aidant principal, ses proches et son entourage soignant, ainsi que les modalit\u00e9s facilitatrices qui seraient acceptables pour envisager des \u00e9changes fluides au sein du trio patient-aidant-soignant concernant la fin, ouvrant un nouveau champ de r\u00e9flexion sur l\u2019\u00e9thique du soin et de la prise en charge de la personne gravement malade, voire mourante.<\/em><\/p>\n<\/div>\n<\/div>\n15h30 : Le bon vieux temps et l\u2019avenir inqui\u00e9tant\u00a0\u00bb : liens entre nostalgie nationale et angoisse existentielle collective\u00a0 ?<\/h4>\n
