Ce projet aborde la question des continuit\u00e9s et discontinuit\u00e9s des soins, accompagnements et liens (sociaux, familiaux et affectifs) dans le cas des \u00e9vacuations sanitaires de mineur.es de Mayotte vers l\u2019Ile de La R\u00e9union. Dans un contexte de fortes disparit\u00e9s dans l\u2019acc\u00e8s aux droits et aux soins, nous analyserons l\u2019exp\u00e9rience des mineur.es et de leur entourage et les modalit\u00e9s de leur accompagnement par les professionnels. Ce projet aborde la question des continuit\u00e9s et discontinuit\u00e9s soignanntes, des accompagnements et liens (sociaux, familiaux et affectifs) dans le cas des \u00e9vacuations sanitaires de mineur.es de Mayotte vers l\u2019Ile de La R\u00e9union.
Le sujet des \u00e9vacuations sanitaires dans les territoires d\u2019Outre-Mer de l\u2019Oc\u00e9an Indien s\u2019inscrit dans une r\u00e9flexion plus g\u00e9n\u00e9rale sur les mobilit\u00e9s et circulations th\u00e9rapeutiques, et sur les exp\u00e9riences de sant\u00e9 et de soin des populations migrantes. A Mayotte, la complexit\u00e9 des parcours de soins des patient.es s\u2019explique autant par leur situation de pr\u00e9carit\u00e9, l\u2019ins\u00e9curit\u00e9 administrative et une offre de soins insuffisante. Dans ce contexte singulier, nous avons choisi d\u2019observer les modalit\u00e9s d\u2019accompagnement de personnes faisant ou ayant fait l\u2019objet d\u2019une \u00e9vacuation sanitaire de Mayotte vers l\u2019Ile de La R\u00e9union et de leur famille – \u00e0 travers l\u2019observation du fonctionnement du dispositif EVASAN et l\u2019analyse du discours des professionnel.les engag\u00e9.es aupr\u00e8s de ces patient.tes. Notre projet vise \u00e0 comprendre les effets de ce contexte (social, g\u00e9opolitique, juridique et institutionnel) sur les trajectoires des mineur.es n\u00e9cessitant des soins et de leurs proches ainsi que les effets de la s\u00e9paration\/rupture pr\u00e9coce sur l\u2019enfant et sa famille lorsque celui-ci part seul, pour un soin de longue dur\u00e9e.<\/p>\n
Le projet poursuit deux principaux objectifs\u00a0:<\/p>\n
1) Identifier et d\u00e9crire les formes et les dispositifs d\u2019accompagnement, officiels ou spontan\u00e9s, et les acteurs impliqu\u00e9s\u00a0: acteurs institutionnels, professionnel.les de sant\u00e9, famille, entourage, associations.<\/p>\n
2)\u00a0 Documenter la diversit\u00e9 des trajectoires des enfants et de leurs proches, \u00e0 l\u2019articulation entre situations sanitaires, configurations familiales, in\u00e9galit\u00e9s de ressources et situations administratives.<\/p>\n
La m\u00e9thode retenue est inductive et compr\u00e9hensive, liant \u00e9troitement savoirs conceptuels et pratiques d\u2019enqu\u00eate ethnographique (entretiens, observations).\u00a0
Notre recherche se construit avec une pluralit\u00e9 de regards du fait de nos trajectoires de recherche inscrites dans diff\u00e9rents champs disciplinaires (anthropologie, sociologie, psychologie, g\u00e9ographie, droit) et nos exp\u00e9riences ant\u00e9rieures de dialogues interdisciplinaires. Cette pluridisciplinarit\u00e9 nous permet de mieux saisir les exp\u00e9riences et trajectoires des enfants et de leurs proches, et les modalit\u00e9s de leur accompagnement\u00a0: nous mettons ainsi en dialogue les approches anthropologiques des circulations th\u00e9rapeutiques, les travaux sociologiques sur les trajectoires de soin, les organisations de sant\u00e9 et les pratiques de soin, la production des in\u00e9galit\u00e9s dans l\u2019enfance, ainsi que les travaux en psychologie sur la pr\u00e9vention des liens pr\u00e9coces dans un contexte de s\u00e9paration.<\/p>\n

Dans les Outre-mer fran\u00e7ais, le dispositif d\u2019\u00e9vacuation sanitaire EVASAN a \u00e9t\u00e9 mis en place pour assurer l\u2019acc\u00e8s effectif de la population \u00e0 la pr\u00e9vention et aux soins sur l\u2019ensemble du territoire. Si ce dispositif constitue une r\u00e9ponse sanitaire \u00e0 des disparit\u00e9s d\u2019offre de soins, il produit n\u00e9anmoins des in\u00e9galit\u00e9s (soins entrav\u00e9s) rep\u00e9rables dans les parcours th\u00e9rapeutiques des patient\u00b7e\u00b7s. Certaines peuvent se traduire par l\u2019\u00e9loignement avec la famille, les proches, l\u2019isolement social des malades, d\u2019autres par des faits de discrimination et de pr\u00e9jug\u00e9s selon la situation administrative des personnes. La journ\u00e9e que nous organisons fera dialoguer chercheur\u00b7e\u00b7s et professionnel\u00b7le\u00b7s sur les enjeux des \u00e9vacuations sanitaires en interrogeant la question de l\u2019accompagnement des personnes malades, plus particuli\u00e8rement celle des in\u00e9galit\u00e9s dans l\u2019acc\u00e8s aux soins de sant\u00e9. Le sujet de l\u2019accompagnement des personnes invite \u00e0 aborder les pratiques des professionnel\u00b7le\u00b7s pr\u00e9sent\u00b7e\u00b7s aupr\u00e8s des patient\u00b7e\u00b7s aux diff\u00e9rentes \u00e9tapes de l\u2019\u00e9vacuation sanitaire, ainsi que le r\u00f4le de leurs proches. La pr\u00e9sence d\u2019un proche aupr\u00e8s du malade faisant l\u2019objet d\u2019une \u00e9vacuation sanitaire n\u2019\u00e9tant pas syst\u00e9matique, y compris lorsque le ou la patient\u00b7e est mineur\u00b7e, nous interrogerons les effets de cette absence sur l\u2019\u00e9tat de sant\u00e9 de l\u2019enfant, les dynamiques familiales et la trajectoire de soin.
